It's all about the relays and clients you use. Some clients allow for great customization of your feeds and their content. I rarely see Mastodon content. Try using some Web of Trust relays and paid relays.

Unfortunately, a lot this is in relays. Relays capture everything on it, I agree there should be a way to curate further at the relay level perhaps but I don't know the best way to do that. I believe the Nostr developers will gradually roll out solutions to all these problems as time goes on. I think I saw a snippet of someone creating a recommend feed today, so it could change sooner than thought.
As long as the nip05s stay standardized it would be extremely simple to filter them. Every dev is complicit in fucking this up
