What do you mean? I suspect the unprovoked stabing of the woman on the train was a victim of hate fueled by racism, but maybe you have a different definition of racism.

Too broad a brush stroke. Its a targeted propaganda subculture problem IMHO. The mental health shit doesn't help.
I don't think it's possible to know that. I also am not 100% sure the murderer was racist. Perhaps there is a 60% chance he hated her because of the color of his skin. Maybe he was delusional. We don't know. Wr also don't know if racism could have prevented this. People can walk in trains. Maybe the voices in his head told him to stab the first blonde he saw. When I saw the video it reminded me to be more aware of my surroundings. I don't want to limit myself to one variable.
It isn't. Racism is what is causing these problems, as some subsets of the population get away with behavior that would get others locked away. Don't need racism to not let the criminally insane roam the streets. Just biased, incompetent government. The government always knows about these people beforehand and just does nothing. This is anarcho-tyranny, where they lock people up for not paying fees or developing software, but let the nutjobs run around, free, because of low expectations.
