yes they do, they get out of sync with the rest of nostr. I spend half my time on here debugging why primal users can't see things.

Can you show some receipts?
they've even purposely censored people from their trending algos (ralf, onyx, semisol). they remove spam. they have complete control of what primal users see. they even manipulate follower counts (intentionally or not), to make it seem their influencers are more popular
