I would assume there are at least thousands of miners in USA alone.
It does not say 40 people mining, it says there are 40 Bitcoin mining sites. Further into the report we clarify these are mines with over 10 MW in power, as anything smaller is difficult to map out.
It also does not say 150 companies are trading. It says there are 150 Bitcoin companies who provide products and services in the Bitcoin space.
