People generally don't value free shit. Some of the dumbest emergency department visits I see are people with free state medical care. Pregnancy tests. STI testing (offered for free at our health department). Free rides (they get a free ride from the hospital to a localish address). Lots of stupid shit like that. Why? Probably because they don't shoulder the burden of it. And the real kick in the nuts is that these people are usually the most abusive to us.
