Can fiat sellers load up so payments can be made instantly? Or would that require too much of you and make you a middleman/transmitter?

No. Payment must be made P2P. But you can try to find a Thai person with cash who will accept bank transfer and you pay with PlebQR. Then you have cash for instant payment. Now all Thai people can be a ATM
