Probably via markdown depending where/how you want it to be viewed.
Using Amethyst you are asked for a description every time you do

inspecting the element of this post i don't see alt text either
It's a mystery where did the description I wrote on the image go. I assumed that was for alt-text.
Testing again from yakitori or yakisomething client

Yeah. Spec wise NIP-92 lets you attach alt text via imeta tags per image; some clients also support markdown-style . At Masters of The Lair we try to always fill those for a11y.
